To my father, Amir-Houshang Movassaghi, MD, whose
kindness and caring energy for others and passion for medicine
forged the way for me. His dedication to fulfilling one’s
potential was a constant source of inspiration. His mantra was,
“When you love your job you don’t work,” and it has always
resonated with me.
To my two sons, Nima and Aria, who I love dearly. They give
me so much joy and have helped me become a better person.
To my best friend and wife, Niloo. She has been my constant
companion, providing wisdom, strength, comfort, support, and
loyalty. Her endless and unconditional love flows effortlessly,
and she enriches the lives of everyone that she meets. At times
this book took me away from my family, but despite late nights,
someone was always waiting to greet me when I came home.
To my patients, who have entrusted me with the sacred
opportunity to help them as their surgeon. It has been truly an
honor to be part of their surgical journey.

Preface
Shaping the Breast grew out of my discontent as a resident and later as a
practicing surgeon with the lack of a precise roadmap to approach the funda-
mentals of breast surgery to achieve optimal results. Despite the vast number
of publications on breast surgery, I felt that there was a deficit in easy-to-
process yet detailed source of information that brought all of the concepts
together. With observation, surgical experience, and better devices, my
approach to breast surgery has evolved from “volumizing” the breast to
“shaping” the breast. This vision was the launching pad for the book.
Chapter 1 is an extensive, in-depth, and detailed presentation of best prac-
tices in breast surgery. It introduces the readers to the process of successful
breast surgery, which includes patient interview, physical analysis, bio-
dimensional planning, surgical marking and execution, and postoperative
care. Chapter 2 builds on the principles learned in Chap. 1 and covers aug-
mentation mastopexy, which is one of the most challenging situations in aes-
thetic breast surgery. Chapter 3 is about the evolving role of fat injection in
breast surgery, with an emphasis on composite breast augmentation. It pro-
vides readers with the realistic expectations that can be achieved by this pro-
cedure, something that is not always fully disclosed. Chapter 4 discusses the
expanding field of breast reconstruction, covering both alloplastic and autolo-
gous breast reconstruction with good references to build on. Lastly, Chap. 5
brings together all the principles learned in the first four chapters and applies
them to the management of very challenging but commonly seen breast cases.
In addition, some technical pearls are introduced in this chapter.
While no single textbook can provide an encyclopedic presentation of
such a broad-based discipline, this book has been designed to provide a foun-

dation of information useful at every level of experience and to stimulate
further self-directed education. The authors have been selected to provide
information based on their experience, enthusiasm, and teaching skills. You
will recognize many as the current leaders in their fields. It has been my privi-
lege to work with this dedicated group of educators and surgeons, and I hope
that the women in your care will benefit from their efforts.

Harvard Combined Plastic Surgery Program. They taught me critical thinking
skills and always encouraged me to push the boundaries of plastic surgery. In

visits to Charles Randquist, MD, in Stockholm, Sweden. He is not only a
