1 1.0 BACKGROUND INFORMATION
2 1.1 PLAN INTRODUCTION AND INTEGRATION
3 This Accident Prevention Plan (APP) and the attached Site Safety and Health Plan
4 (SSHP) have been developed for the US Army Tank-automotive and Armaments
5 Command (TACOM) and the Base Realignment and Closure Technical Support Office
6 (BRACO). This APP has been developed to meet the US Army requirements as outlined
7 in the Section 01.A.09 of the US Army Corps of Engineers (USACE) Engineering Manual
8 (EM) 385-1-1, Safety Manual. EM 385-1-1 is the primary US Army safety guidance
9 document to which all removal of buildings, concrete floor slabs and foundations at
10 RVAAP-08 Load Line 1 (LL1) and other miscellaneous buildings and removal and
11 disposal of pallets activities will adhere.

13 Since this APP will also be applied to Military Munitions Response Program (MMRP)
14 sites, the APP and the attached SSHP have been developed in accordance with (IAW)
15 the US Army Engineering and Support Center, Huntsville (CEHNC) Data Item
16 Description (DID) MR-005-06. This APP will act as PIKA’s Environmental, Safety and
17 Health (ES&H) document for this project, with the SSHP being used to contain the site-
18 specific data and procedures.

20 As stated in the MR-005-06, “The APP shall be an implementing document with
21 emphasis on “who” will have each of the specific responsibilities and “how” and “when”
22 each of the applicable requirements will be performed”. However, the historical intent of
23 an SSHP has been to provide a document for the field personnel that effectively
24 summarized the hazards associated with the project and the procedures and controls
25 that will be used to mitigate and when possible eliminate the hazards. As such, where
26 duplication exists between the APP requirements and the SSHP, PIKA has chosen to
27 present the bulk of the ES&H control data in the SSHP presented in Attachment 1 of
28 this APP. Elements of this APP that are better addressed in the SSHP have been
29 identified in this APP and a statement has been included that specifies where in the
30 SSHP the required element can be found. Conversely, if an element is better suited for
31 presentation in the APP, it has been addressed in this document and a statement has
32 been inserted in the SSHP indicating that the element is presented in the APP.

2 The contractor for the performance of this project is PIKA. As the contractor for this
3 Task Order (TO), PIKA will be responsible for its successful completion and for the
4 management of all resources required to meet the requirements of the SOW.

8 The name of this project is the Removal of Buildings, Concrete Floor Slabs and
9 Foundations at LL1, & other Miscellaneous Buildings and Removal & Disposal of Pallets.
10 1.5 BRIEF PROJECT DESCRIPTION
11 1.5.1 Description of Work to be Performed
12 The objective of this project is to demolish, remove and certify 5X the RVAAP LL1
13 building slabs and foundations, buildings F15 and F16 floor slabs and foundations,
14 buildings G1, G1A, G3, CB13, CB801 and EB803 contents, superstructures, and floor
15 slabs and foundations and removal and disposal of wooden pallets staged within various
16 RVAAP Group 2 buildings. To meet the objectives of the SOW, PIKA will conduct the
17 following general tasks:
18 • Survey the corners of all buildings and sumps within LL1 and buildings F15 and
19 F16 using high resolution GPS unit to chronicle the locations for future reference
20 and environmental restoration projects.
21 • Remove and dispose of all floor slabs and foundations to a minimum of one foot
22 below ground surface of all buildings in LL1 except Change House buildings
23 CB22, CB23 and CB12. Maintain records of the location and estimated amount of
24 any remaining foundation.
25 • Remove and dispose of all masonry, unused telephone poles, steam stanchions,
26 electric utility boxes and any miscellaneous wood and steel located within the
27 perimeter fence of LL1 in accordance with (IAW) all applicable federal, state and
28 local rules, laws and regulations. Ensure that all holes resulting from pole
29 removal are filled to eliminate any potential safety hazard. Any missing manhole
30 covers will be replaced with temporary covers fabricated from ¾-inch treated
31 plywood or other similar semi durable material.
32 • Remove and dispose of floor slabs and foundations to a minimum of one foot
33 below grade and dispose of any miscellaneous concrete, steel and wood at
